8. You visit to assess an elderly bed bound lady being cared for by her husband. On examination you discover what appears to be a Grade 2 pressure ulcer on her sacrum. What is your plan of action for this lady?
Complete a full risk & skin assessment to include the grade, size and extent of pressure ulcer, using a validated classification tool and measurement technique as per TVN /SSKIN policy
Put in place appropriate pressure redistribution devices to prevent further pressure damage
Treat wound according to tissue viability policy taking into consideration pain tolerance of patient, position of ulcer, amount of exudate and frequency of dressing change
Offer information to the patient and husband the causes, early signs, prevention and treatment of pressure ulcers and the plan of care for the patient
Assess nutritional and fluid intake offering advice on how to follow a balanced diet to maintain adequate nutritional status if required possible need for GP / Dietician referral
